Why Medlog Health

The Medlog Health platform enables long-term, post-acute care (LTPAC) practices to marry their in-person care model with asynchronous, online care delivery.

 

Our goal is to enable continuous, physician-led care coordination and communication for nursing facility residents. There is growing evidence that increasing physician involvement between in-person visits can bring about positive outcomes for patients, as well as reduce healthcare costs. When physicians are incentivized to quickly address patient needs, patients can receive the care they need without having to visit urgent care or emergency departments. We believe asynchronous care delivery can drive improvements in nursing home care while also providing additional revenue for practices.

The problem with providing asynchronous nursing facility care today is the lack of purpose-built technologies for physicians who practice in these settings. Nursing home residents are typically not able or lack the desire to learn how to use digital health applications to interact with their care team. Instead, on-site clinical staff are available to collect patient histories, perform physical exams and relay information to the physician who is typically not in the building.

Medlog supports asynchronous care delivery through SmartText Visits. The foundation of SmartText Visits are problem-specific, physician-designed question sets that on-site nurses fill out on the Medlog Health app. This ensures a strong, thorough initial history and exam without the physician having to repeatedly type out questions or wait for each individual response. Medlog has spent hundreds of hours building custom question sets for commonly encountered chief complaints in nursing facility settings. With SmartText, physicians receive actionable information immediately to start their clinical decision-making resulting in quicker resolution for patients and a better experience for clinicians.

SmartText Visit vs Traditional Messaging

Medlog Health is an innovative platform that gives residents access to always-on, quality care while offering physicians more efficient ways to receive compensation for comprehensive care.

Most LTPAC physicians are already inundated with text messages from facility staff related to patient issues. These physicians have learned that performing this asychronous care using nothing but secure texting is slow and painful. The lack of thorough information conveyed over traditional text messaging, HIPAA-compliant or otherwise, leads to time-consuming back and forth and makes it difficult to document the care provided. With SmartText, nurses can take as long as needed to fully complete their history without slowing down the physician. SmartText also facilitates medical thoroughness and automated documentation protecting physicians medicolegally and eliminating duplicative work. Finally, Medlog makes it easy for practices to capture SmartText Visits and bill for them using our proprietary rules engine. The result is a clean set of claims each month that are delivered to the practice.
